1. Outstanding balances must be paid before we can accept a new application. Please contact the finance director if you have a question about a balance. 2. All members must have an immunization record and a copy of a physical that has taken place on or after June 26, 2011 on file or included with application. Your doctor may fax this information to us at 229-0305. Not sure if you are up to date? Contact the membership director. 3. Complete the application form, except for the payment contract on the last page. 4. Meet with a staff member at any one of our sites to review the payment contract. Applications cannot be faxed in. Just drop by Bradley Street between 8 am and 6 pm or Warner, Hopkinton or Suncook between 3 and 6 pm. If you wish to set up an appointment, please call or email AshleyJane Kneeland. 5. Current recipients of State of New Hamphire Childcare Assistance can be linked to the Club using Form 2530; please discuss at your one-on-one meeting. If you wish to apply for financial assistance from us, please bring documentation of income. We'll accept your most recent tax return or three paystubs or social security stubs/letter. 6. Submit the application form (and physical and immunization forms if necessary) along with a non-refundable $10 application fee. |
