| How to Register1) Be up to date on past summer or afterschool program payments if applicable.
2) Fill out a summer camp application. 3) Obtain camper's immunization record and medical form of appointment dated 8/27/10 or later (if you think you have one on file, please contact AshleyJane Kneeland. These forms can be mailed or faxed to AshleyJane's attention at 229-0305. 4) Stop by the Bradley Street Unit between 8 am and 4 pm or other units between 2:30 and 6 pm to review and sign the payment contract ALONG WITH A STAFF MEMBER. Other times may be arranged by calling 224-1061. 5) If you are applying for a Club scholarship, please bring proof of income to your payment contract review. We accept tax returns, paystubs and/or social security stubs. 6) If you have State of NH Childcare Assistance, you can pick up the necessary forms at your payment contract review. |
